Vendor needs to provide care plan management system for skilled nursing facilities.
- Obtain a cost-effective enterprise solution that supports electronic reporting of resident care.
- Improve resident care by providing a user-centric platform that will integrate progress notes,
assessments, documentation records, resident management and clinical administration tools as needed.
- Provide clinical libraries to enable the production of care plans that meet all established
evidence-based practice guidelines.
- Provide a tool that can generate high quality care plans that can be modified and customized as
needed based upon plans within the clinical libraries.
- Improve reporting and analytical capabilities to report on the quality and cost of resident care in
DSOHF facilities.
- Ensure adherence to regulatory compliance with current standards and regulations.
- Reduce the administrative burden team members face by automating routine resident
documentation, thereby leaving more time for staff to focus on the quality of care they are
providing.
- Drastically reduce time spent on document creation, maintenance and population by using
standardized templates.
- Eliminate admission documentation errors that result from manually completing forms by pre--
populating electronic forms with the correct data.
- Achieve better oversight of current documentation across the organization, ensuring compliance
and standardization of required documentation.
- Ability to update standard documentation immediately, thereby reducing wait times for copies to
be distributed and customized at each site.
- Enable quick and confident decision making with real-time access to complete resident charts
and provide vivid analytic tools.
- Ability to identify trends, intervene early, and improve resident outcomes with intelligent alerts
and reporting tools.

Performance of the work will be Offsite. Vendor needs to carry work in their office premises.
