[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide adult case management services including assessment, planning, facilitation and advocacy for options and services that meet an individual physical and behavioral health needs and services require initial and ongoing determination of client eligibility as well as development of written service plans, both scheduled and non-scheduled client contacts occur with home visits required at a minimum of once per quarter and a full client reassessment required annually, all client contacts must be documented; and must include information regarding the clients condition, family and community support system, progress made toward goals, and the ongoing ability of the client to maintain self-sufficiency to the government authority located in Raleigh, NC.
(2) All question must be submitted no later than July 20, 2018.

Onshore (USA Only)
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
